Informed Consent to Telehealth Services

Boudica Agency LLC d/b/a Youth & Soul ("Youth & Soul") operates a technology platform that connects adults in the United States with independent, US-licensed healthcare providers ("Providers") for telehealth visits. By using the Services, you acknowledge that you have read, understood, and agreed to this Informed Consent to Telehealth Services.
1. What telehealth is
Telehealth uses electronic communications — including secure messaging, online intake forms, photographs, audio, and video — to deliver healthcare services when the patient and the provider are in different locations. Through Youth & Soul, your visit is conducted asynchronously and/or by live communication with a Provider licensed in the state where you are located.
2. Expected benefits
- Access to licensed clinicians without traveling to a physical office.
- Convenient and private evaluation of certain non-emergency conditions.
- Ongoing care coordination through your secure account.
3. Possible risks and limitations
- The Provider cannot perform a hands-on physical examination. Information available to the Provider may be limited compared to an in-person visit.
- The Provider may determine that telehealth is not appropriate for your condition and recommend in-person evaluation or refer you to emergency care.
- Technology failures, connection issues, or transmission errors may interrupt or delay care.
- Despite reasonable security measures, electronic communications carry inherent risks of unauthorized access or disclosure.
- Treatment, including any prescription, is at the sole clinical discretion of the Provider. Payment for a consultation does not entitle you to a specific treatment or prescription.
4. Not for emergencies
The Services are not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Do not use Youth & Soul to seek urgent or emergent care.
5. Your responsibilities
- Provide complete, accurate, and current information during intake and follow-up.
- Disclose all medications, supplements, allergies, and relevant medical history.
- Follow the Provider's instructions and report adverse reactions promptly.
- Maintain a primary care provider for in-person care when needed.
6. Privacy and records
Information you share during a telehealth visit is protected under HIPAA and applicable state laws. See our HIPAA Notice of Privacy Practices for details on how Protected Health Information is used and disclosed. Visit records become part of your medical record.
7. Right to refuse or withdraw consent
Your participation is voluntary. You may refuse or withdraw consent to telehealth at any time, without affecting your right to future care, by contacting your Provider or support@youthandsoul.com. Withdrawing consent does not affect care already provided.
8. Acknowledgment
By creating an account, completing an intake, or otherwise using the Services, you confirm that you are at least 18 years old, a resident of the United States, located in a state where the Provider is licensed at the time of the visit, and that you consent to receive care via telehealth on the terms above.
